Summary
Levoketoconazole strongly inhibits CYP3A4 and itself prolongs the QT interval. Cisapride concentrations rise and the risk of torsades de pointes and cardiac arrest increases substantially.
What this interaction means in plain words
Cisapride and Levoketoconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Cisapride belongs to Gastrointestinal Tract; Levoketoconazole belongs to Endocrine.
Why it happens
Strong CYP3A4 inhibition reduces cisapride metabolism, combined with additive QT prolongation.
What to do
Do not coadminister; use an alternative prokinetic during levoketoconazole therapy.
