Levoketoconazole is a strong CYP3A4 inhibitor and markedly raises tolvaptan exposure. Strong CYP3A inhibitors are contraindicated with tolvaptan because of the risk of overly rapid sodium correction and other toxicity.
What this interaction means in plain words
Levoketoconazole and Tolvaptan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Why it happens
Levoketoconazole strongly inhibits CYP3A4, greatly reducing tolvaptan clearance.
