Summary

Zolmitriptan is partly cleared through MAO-A, and its active metabolite as well, so linezolid can raise exposure while adding serotonergic effect. This increases the risk of serotonin syndrome.

What this interaction means in plain words

Linezolid and Zolmitriptan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Linezolid belongs to Anti Bacterial; Zolmitriptan belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Inhibition of MAO-A mediated metabolism of zolmitriptan plus additive serotonergic effects.

What to do

Avoid zolmitriptan during linezolid therapy and for 2 weeks after MAOI exposure as labeled; use an alternative acute migraine therapy and monitor for serotonin toxicity.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
