Summary
Liraglutide adds glucose-lowering effect to saxagliptin and metformin through complementary incretin actions. Hypoglycemia is uncommon with this combination alone but becomes more likely if a sulfonylurea or insulin is also present. Duplicate incretin-based therapy with a DPP-4 inhibitor and a GLP-1 agonist offers little added benefit.
What this interaction means in plain words
Liraglutide and Saxagliptin-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Liraglutide belongs to Endocrine; Saxagliptin-metformin belongs to Endocrine.
Why it happens
Additive pharmacodynamic effect, as both liraglutide and saxagliptin enhance incretin signaling while metformin lowers hepatic glucose output.
What to do
Generally avoid combining a DPP-4 inhibitor with a GLP-1 receptor agonist and consider stopping saxagliptin. If used together, monitor glucose and reduce any concurrent sulfonylurea or insulin dose.
