Summary
Systemic pentamidine can prolong the QT interval and cause torsades de pointes, and lithium may add to this effect. Pentamidine may also cause hypoglycemia and electrolyte disturbances that further increase arrhythmia risk.
What this interaction means in plain words
Lithium and Pentamidine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lithium belongs to Central Nervous System; Pentamidine belongs to Anti Parasitic.
Why it happens
Additive QTc prolongation, with electrolyte effects from pentamidine amplifying the risk.
What to do
Check a baseline ECG and electrolytes and monitor the ECG during pentamidine treatment. Correct potassium and magnesium, and consider an alternative anti-infective if possible.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
