Summary

Lomitapide inhibits P-glycoprotein and may raise vinblastine exposure, increasing the risk of myelosuppression and neurotoxicity. Vinblastine is a weak CYP3A4 substrate and any effect on lomitapide exposure is not well established, so the clinical relevance is mainly in the vinblastine direction.

What this interaction means in plain words

Lomitapide and Vinblastine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Lomitapide belongs to Cardiovascular; Vinblastine belongs to Oncology.

Evidence level: Low

Why it happens

Lomitapide inhibits P-glycoprotein efflux of vinblastine, which may increase vinblastine systemic exposure.

What to do

Monitor blood counts and signs of neurotoxicity or gastrointestinal toxicity, and consider reducing the vinblastine dose if toxicity appears. Monitor liver enzymes as usual for lomitapide.

What to ask your pharmacist
