Summary

Metoclopramide is cleared mainly through CYP2D6, and inhibition of this enzyme raises its exposure. Higher levels increase the risk of extrapyramidal symptoms, tardive dyskinesia and sedation. Lorcaserin is a weak CYP2D6 inhibitor and also has serotonergic activity, so the effect is modest.

What this interaction means in plain words

Lorcaserin and Metoclopramide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

CYP2D6 inhibition by lorcaserin reduces metoclopramide metabolism, with possible additive serotonergic and dopaminergic adverse effects.

What to do

Monitor for extrapyramidal symptoms, restlessness and sedation, and lower the metoclopramide dose if they appear. Keep the metoclopramide course short.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
