Summary

Verapamil is a moderate CYP3A4 inhibitor and raises lurasidone exposure, which can increase sedation, akathisia, extrapyramidal symptoms and other dose-related adverse effects. Lurasidone may also add to the blood pressure lowering of verapamil, leading to hypotension or orthostasis.

What this interaction means in plain words

Lurasidone and Verapamil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Lurasidone belongs to Central Nervous System; Verapamil belongs to Cardiovascular.

Evidence level: High

Why it happens

Verapamil inhibits intestinal and hepatic CYP3A4, reducing lurasidone clearance, and both drugs have additive hypotensive effects.

What to do

Start lurasidone at a reduced dose and keep within the labeled maximum for use with moderate CYP3A4 inhibitors. Monitor for extrapyramidal symptoms, sedation and blood pressure.
