Summary
Loop diuretics such as torsemide increase urinary magnesium excretion and can lead to hypomagnesemia over time, although torsemide has a smaller effect than furosemide. Low magnesium raises the risk of arrhythmias and refractory hypokalemia.
What this interaction means in plain words
Magnesium and Torsemide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Magnesium belongs to Gastrointestinal Tract; Torsemide belongs to Cardiovascular.
Why it happens
Inhibition of the Na-K-2Cl cotransporter in the thick ascending limb reduces paracellular magnesium reabsorption.
What to do
Monitor serum magnesium and potassium periodically in patients on long-term torsemide, and supplement magnesium if levels are low.
