Summary

Ribociclib causes concentration-dependent QT prolongation, and maprotiline can also prolong the QT interval. Co-administration increases the risk of torsades de pointes. Ribociclib also inhibits CYP3A4 and may modestly affect other drugs.

What this interaction means in plain words

Maprotiline and Ribociclib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Maprotiline belongs to Central Nervous System; Ribociclib belongs to Oncology.

Evidence level: Moderate

Why it happens

Additive QT prolongation through effects on cardiac repolarization.

What to do

Avoid the combination and select an antidepressant with less QT effect. If it cannot be avoided, monitor ECG and electrolytes at baseline and during treatment, and interrupt or reduce ribociclib if QTc becomes markedly prolonged.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
