Summary
Ticlopidine inhibits CYP2C19, which is the main route of mavacamten metabolism, and may raise its exposure. Excess mavacamten can reduce ejection fraction and lead to heart failure from systolic dysfunction. The labeling contraindicates moderate to strong CYP2C19 inhibitors.
What this interaction means in plain words
Mavacamten and Ticlopidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Mavacamten belongs to Cardiovascular; Ticlopidine belongs to Cardiovascular.
Why it happens
Inhibition of CYP2C19 by ticlopidine reduces mavacamten clearance.
What to do
Avoid the combination and choose an alternative antiplatelet drug if possible. If it cannot be avoided, monitor LVEF closely and consider mavacamten dose reduction or interruption.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
