Summary
Ribociclib can prolong the QT interval, and donepezil has also been associated with QT prolongation and torsades de pointes. Ribociclib inhibits CYP3A4, which may modestly raise donepezil exposure and add to the arrhythmic risk.
What this interaction means in plain words
Memantine-donepezil and Ribociclib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Memantine-donepezil belongs to Central Nervous System; Ribociclib belongs to Oncology.
Why it happens
Additive QT prolongation plus CYP3A4 inhibition by ribociclib that may increase donepezil concentrations.
What to do
Avoid the combination where possible; if it is unavoidable, obtain a baseline ECG and monitor it along with electrolytes (potassium, magnesium). Correct electrolyte abnormalities and stop or switch therapy if QTc becomes markedly prolonged.
What to ask your pharmacist

- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
