Summary
Methylprednisolone is cleared by CYP3A4, and azole inhibitors can substantially raise its exposure. Miconazole absorbed from vaginal use produces a smaller effect. Increased exposure can lead to Cushingoid effects, hyperglycemia and immunosuppression.
What this interaction means in plain words
Methylprednisolone and Miconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Methylprednisolone belongs to Endocrine; Miconazole belongs to Anti Fungals.
Why it happens
Miconazole inhibits CYP3A4, decreasing methylprednisolone clearance.
What to do
Monitor for corticosteroid excess such as hyperglycemia, fluid retention and infection. Consider lowering the methylprednisolone dose if prolonged or systemic miconazole is needed.
