Summary
Propafenone inhibits CYP2D6 and substantially raises metoprolol concentrations, in addition to having its own beta-blocking and negative inotropic effects. This can lead to marked bradycardia, hypotension and heart failure worsening.
What this interaction means in plain words
Metoprolol and Propafenone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Metoprolol belongs to Cardiovascular; Propafenone belongs to Cardiovascular.
Why it happens
CYP2D6 inhibition reducing metoprolol clearance, with additive beta-blocking and conduction effects.
What to do
Monitor heart rate, blood pressure and ECG, and reduce the metoprolol dose as needed. Consider an alternative beta-blocker not dependent on CYP2D6.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
