Summary

Cytokine release syndrome caused by talquetamab can temporarily suppress CYP enzyme activity and raise mexiletine exposure. This may cause mexiletine toxicity such as tremor, dizziness, nausea or arrhythmia, particularly during step-up dosing and the following days.

What this interaction means in plain words

Mexiletine and Talquetamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Mexiletine belongs to Cardiovascular; Talquetamab belongs to Oncology.

Evidence level: Low

Why it happens

Cytokine-mediated downregulation of CYP1A2 and CYP2D6 reduces mexiletine metabolism.

What to do

Monitor for mexiletine adverse effects and ECG changes from the start of step-up dosing until about 14 days after the first treatment dose and during any CRS. Reduce the mexiletine dose if toxicity appears.

What to ask your pharmacist
