Summary
Mifepristone inhibits CYP3A4, which markedly raises simvastatin exposure, and may also affect hepatic uptake transporters. This increases the risk of myopathy and rhabdomyolysis, and simvastatin labeling contraindicates its use with strong CYP3A4 inhibitors.
What this interaction means in plain words
Mifepristone and Simvastatin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Mifepristone belongs to Endocrine; Simvastatin belongs to Cardiovascular.
Why it happens
CYP3A4 inhibition (and possible OATP1B1 inhibition) by mifepristone increases simvastatin concentrations.
What to do
Do not combine; use a statin less dependent on CYP3A4 such as pravastatin or rosuvastatin at an appropriate dose. Counsel on muscle symptoms.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?

Frequently asked questions
Can I take Mifepristone with Simvastatin?
MediHelp rates the interaction as contraindicated. Mifepristone and Simvastatin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
How serious is the interaction between Mifepristone and Simvastatin?
MediHelp rates it contraindicated, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Do not combine; use a statin less dependent on CYP3A4 such as pravastatin or rosuvastatin at an appropriate dose. Counsel on muscle symptoms.
