Summary

NSAIDs increase the risk of bleeding in warfarin-treated patients, particularly gastrointestinal bleeding. Nabumetone is relatively COX-2 preferential but the risk is still clinically relevant.

What this interaction means in plain words

Nabumetone and Warfarin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Nabumetone belongs to Musculoskeletal; Warfarin belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAIDs impair platelet function and damage the gastroduodenal mucosa, adding to the anticoagulant effect of warfarin; some may also displace warfarin from protein binding.

What to do

Avoid if possible and use paracetamol or another alternative for analgesia. If needed, use the lowest dose for the shortest time, monitor INR and consider gastroprotection with a proton pump inhibitor.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
