Summary
Combining an opioid agonist-antagonist with a benzodiazepine hypnotic produces additive central nervous system depression. This can cause profound sedation, respiratory depression, coma and death, particularly in older or debilitated patients and those with lung disease.
What this interaction means in plain words
Nalbuphine and Temazepam can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Nalbuphine belongs to Musculoskeletal; Temazepam belongs to Central Nervous System.
Why it happens
Additive CNS and respiratory depression through opioid receptor activity and enhanced GABA-A receptor signalling.
What to do
Reserve concurrent use for patients without alternatives, using the lowest effective doses and shortest duration. Monitor respiratory rate and level of sedation, and counsel patients to avoid alcohol and driving.
