Summary
Nefazodone is a CYP3A4 substrate and a potent CYP3A4 inhibitor, and posaconazole inhibits CYP3A4 as well, so nefazodone exposure may rise with sedation, hypotension and other adverse effects. Posaconazole exposure may also increase modestly.
What this interaction means in plain words
Nefazodone and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Mutual CYP3A4 inhibition, with possible P-glycoprotein effects, reduces clearance of both drugs.
What to do
Avoid the combination where possible; if used, start nefazodone at a low dose and monitor for excess sedation, hypotension, QT-related symptoms and posaconazole adverse effects.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
