Summary
Ranolazine is a CYP3A4 substrate and is also a weak inhibitor of CYP3A4 and P-gp, while nifedipine is only a weak CYP3A4 inhibitor. Any increase in ranolazine exposure is expected to be small, but the combination adds antianginal effect and may increase dizziness or hypotension.
What this interaction means in plain words
Nifedipine and Ranolazine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Nifedipine belongs to Cardiovascular; Ranolazine belongs to Cardiovascular.
Why it happens
Weak CYP3A4 inhibition by nifedipine may slightly raise ranolazine levels, with additive hemodynamic effects.
What to do
No dose adjustment is usually needed. Monitor blood pressure and QT-related symptoms, and check for dizziness.
