Summary

Nilotinib inhibits CYP3A4 and P-glycoprotein and can raise ranolazine levels. Both drugs prolong the QT interval, so the combination raises the risk of torsades de pointes and sudden death.

What this interaction means in plain words

Nilotinib and Ranolazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Nilotinib belongs to Oncology; Ranolazine belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QT prolongation with CYP3A4 and P-glycoprotein inhibition by nilotinib increasing ranolazine exposure.

What to do

Do not combine; choose a non-QT-prolonging antianginal during nilotinib therapy. If unavoidable, obtain ECG monitoring and correct electrolytes.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
