Summary

Phenothiazines block alpha-adrenergic receptors and can blunt the pressor response to norepinephrine. Higher vasopressor doses may be needed, and hypotension may be more difficult to correct. Arrhythmia risk from the sympathomimetic may also be additive in predisposed patients.

What this interaction means in plain words

Norepinephrine and Prochlorperazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism via alpha-1 adrenergic receptor blockade by prochlorperazine.

What to do

Titrate norepinephrine to blood pressure response with continuous hemodynamic monitoring. Hold prochlorperazine in patients who are in shock if possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
