Summary

Posaconazole is a strong CYP3A4 inhibitor, and paritaprevir and ritonavir are CYP3A substrates, so exposure to the antiviral regimen may rise. Higher paritaprevir levels may increase the risk of hepatic and other adverse effects, and ritonavir may in turn raise posaconazole exposure.

What this interaction means in plain words

Ombitasvir / paritaprevir / ritonavir and dasabuvir and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Posaconazole inhibits CYP3A4-mediated metabolism of paritaprevir and ritonavir, and ritonavir inhibits CYP3A4 metabolism of posaconazole.

What to do

Avoid the combination where an alternative antifungal or HCV regimen exists. If it is unavoidable, monitor liver function and for adverse effects of both agents.
