Summary
Botulinum toxin weakens muscle by blocking acetylcholine release, and neuromuscular blockers can add to this effect. Excess weakness, including swallowing or breathing difficulty, may occur, especially if toxin has spread systemically.
What this interaction means in plain words
OnabotulinumtoxinA and Pancuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
OnabotulinumtoxinA belongs to Musculoskeletal; Pancuronium belongs to Musculoskeletal.
Why it happens
Additive neuromuscular blockade: presynaptic inhibition of acetylcholine release by the toxin plus postsynaptic nicotinic receptor blockade by pancuronium.
What to do
Tell the anesthesia team about recent botulinum toxin injections. Titrate pancuronium with train-of-four monitoring and watch for prolonged weakness or respiratory compromise.
