Summary

Perphenazine is a phenothiazine with sedative properties and adds to opioid-induced CNS and respiratory depression, with possible hypotension. Its CYP2D6 inhibition has little relevance for oxymorphone, which is mainly cleared by glucuronidation, so the sedative effect dominates.

What this interaction means in plain words

Oxymorphone and Perphenazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Oxymorphone belongs to Musculoskeletal; Perphenazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive CNS depression and hypotensive effects of the antipsychotic and the opioid.

What to do

Start with lower doses of oxymorphone and titrate while monitoring sedation, respiratory rate and blood pressure. Avoid additional CNS depressants where possible.
