Summary

Probenecid reduces the renal tubular secretion of NSAIDs and can raise piroxicam concentrations, increasing the risk of gastrointestinal and renal adverse effects. Piroxicam may also work against the uricosuric purpose of probenecid to a minor extent.

What this interaction means in plain words

Piroxicam and Probenecid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Piroxicam belongs to Musculoskeletal; Probenecid belongs to Genitourinary.

Evidence level: Moderate

Why it happens

Probenecid inhibits organic anion transporters in the renal tubule, reducing piroxicam clearance.

What to do

Monitor for NSAID toxicity such as dyspepsia, GI bleeding and fluid retention, and use the lowest effective piroxicam dose. Consider a lower piroxicam dose or a different NSAID.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
