Summary
Posaconazole can lower serum potassium and magnesium, and bowel-preparation solutions cause fluid and electrolyte shifts. Electrolyte disturbances together with posaconazole's QT-prolonging potential raise the risk of ventricular arrhythmias, particularly in patients with other QT risk factors.
What this interaction means in plain words
Posaconazole and Sodium sulfate / potassium chloride / magnesium sulfate / polyethylene glycol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive electrolyte loss (hypokalemia/hypomagnesemia) from bowel cleansing on top of posaconazole-associated hypokalemia, which increases susceptibility to QT-related arrhythmia.
What to do
Check potassium and magnesium before and after bowel preparation and correct deficits before continuing posaconazole. Obtain an ECG in patients with cardiac disease or other QT-prolonging drugs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
