Summary

Vincristine is a CYP3A4 and P-gp substrate, and posaconazole inhibits both. Raised vincristine exposure has led to severe neurotoxicity, constipation or ileus and SIADH.

What this interaction means in plain words

Posaconazole and Vincristine liposomal are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Posaconazole inhibits CYP3A4 and P-glycoprotein, reducing vincristine clearance.

What to do

Avoid the combination and use a non-azole antifungal such as an echinocandin during vincristine therapy. If exposure occurs, monitor closely for neuropathy and bowel dysfunction.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
