Summary
Primidone is metabolized to phenobarbital and is sedating, so combined use with risperidone increases CNS depression. Primidone-derived enzyme induction may also lower risperidone concentrations and reduce antipsychotic effect.
What this interaction means in plain words
Primidone and Risperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Primidone belongs to Central Nervous System; Risperidone belongs to Central Nervous System.
Why it happens
Additive CNS depression, plus CYP3A4 and P-glycoprotein induction by primidone and its phenobarbital metabolite.
What to do
Monitor sedation and psychiatric response, and increase the risperidone dose if effect is lost. Reduce it again if primidone is stopped.
What to ask your pharmacist
