Summary
Primidone is metabolized to phenobarbital and both cause sedation, which adds to tramadol's central depressant effects and can lead to respiratory depression. Enzyme induction can also lower tramadol and its active metabolite levels and reduce analgesia. Tramadol may lower the seizure threshold in patients treated for epilepsy.
What this interaction means in plain words
Primidone and Tramadol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Primidone belongs to Central Nervous System; Tramadol belongs to Musculoskeletal.
Why it happens
Additive CNS depression plus CYP3A4/CYP2B6 induction by primidone and its metabolite phenobarbital that increases tramadol clearance.
What to do
Prefer a non-tramadol analgesic in patients with epilepsy; if used, monitor sedation, seizure control and analgesic response. Titrate tramadol cautiously.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
