Summary
Procarbazine has monoamine oxidase inhibitor activity, and MAO-A is the main enzyme that clears zolmitriptan. Combined use raises zolmitriptan exposure and adds serotonergic effects, which can lead to serotonin syndrome and excessive triptan effects.
What this interaction means in plain words
Procarbazine and Zolmitriptan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Procarbazine belongs to Oncology; Zolmitriptan belongs to Central Nervous System.
Why it happens
MAO-A inhibition by procarbazine reduces zolmitriptan metabolism and adds to serotonergic activity.
What to do
Do not give zolmitriptan during procarbazine therapy or within 2 weeks of stopping it. Use a non-MAO-metabolized alternative for migraine and watch for serotonin toxicity if exposure occurs.
