Summary

Both drugs block muscarinic receptors, so combining them can add to anticholinergic effects such as dry mouth, constipation, urinary retention, blurred vision and tachycardia. Inhaled umeclidinium has low systemic exposure, so the effect is usually modest, but it can matter in older patients or those with glaucoma or prostatic hyperplasia.

What this interaction means in plain words

Propantheline and Umeclidinium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic antagonism of muscarinic acetylcholine receptors.

What to do

Monitor for anticholinergic adverse effects such as urinary retention, constipation and dry mouth. Reduce or stop propantheline if symptoms appear, especially in elderly patients or those with narrow-angle glaucoma.

What to ask your pharmacist
