Summary

IL-6 suppresses CYP450 enzymes during inflammation, and sarilumab can reverse this and restore enzyme activity. Quinidine concentrations may therefore fall after sarilumab is started, with possible loss of effect. The reverse may occur when sarilumab is stopped.

What this interaction means in plain words

Quinidine and Sarilumab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Quinidine belongs to Anti Parasitic; Sarilumab belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

IL-6 receptor blockade normalizes suppressed CYP3A4 expression and increases quinidine metabolism.

What to do

Monitor quinidine concentrations and the ECG when sarilumab is started or stopped. Adjust the quinidine dose based on levels and response.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
