Summary

Talquetamab causes cytokine release, which temporarily suppresses CYP450 enzymes and can raise concentrations of CYP substrates such as quinidine. Quinidine toxicity, including QT prolongation, may result, especially around step-up dosing and during cytokine release syndrome.

What this interaction means in plain words

Quinidine and Talquetamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Quinidine belongs to Anti Parasitic; Talquetamab belongs to Oncology.

Evidence level: Low

Why it happens

Cytokine-mediated suppression of CYP3A4 reduces quinidine metabolism.

What to do

Monitor ECG and quinidine levels during talquetamab step-up dosing and for about two weeks after, and during any cytokine release syndrome. Reduce the quinidine dose if concentrations or QTc rise.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
