Summary
Vilazodone is a serotonin reuptake inhibitor and 5-HT1A partial agonist, and selegiline is a monoamine oxidase inhibitor that at antidepressant doses is not selective. Combining them can produce serotonin syndrome with hyperthermia, agitation, myoclonus, autonomic instability and seizures.
What this interaction means in plain words
Selegiline transdermal and Vilazodone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Additive serotonergic activity from serotonin reuptake inhibition plus inhibition of serotonin breakdown by MAO.
What to do
Do not coadminister. Allow at least 14 days between stopping an MAOI and starting vilazodone, and at least 14 days after stopping vilazodone before starting selegiline.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
