Summary
Both drugs prolong the QT interval, and vandetanib carries a boxed warning for QT prolongation, torsades de pointes and sudden death. Combined use greatly increases the risk of life-threatening ventricular arrhythmia.
What this interaction means in plain words
Sotalol and Vandetanib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Sotalol belongs to Cardiovascular; Vandetanib belongs to Oncology.
Why it happens
Additive blockade of cardiac potassium channels (hERG) causing delayed repolarization and QT prolongation.
What to do
Avoid the combination; use a non-QT-prolonging antiarrhythmic or rate-control alternative. If it cannot be avoided, correct potassium and magnesium and perform frequent ECG monitoring.
What to ask your pharmacist
