Summary

Both vardenafil and tacrolimus can prolong the QT interval, so using them together may add to the risk of ventricular arrhythmia including torsades de pointes. The risk is greatest in patients with electrolyte disturbances, structural heart disease or other QT-prolonging drugs.

What this interaction means in plain words

Tacrolimus and Vardenafil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Tacrolimus belongs to Miscellaneous; Vardenafil belongs to Genitourinary.

Evidence level: Low

Why it happens

Additive pharmacodynamic QT prolongation through effects on cardiac repolarization.

What to do

Obtain a baseline ECG in at-risk patients and correct potassium and magnesium before combining. Monitor the QTc and avoid adding other QT-prolonging agents.

What to ask your pharmacist
