Summary

Tranylcypromine is an irreversible MAO inhibitor, and with tapentadol it can cause hypertensive crisis, serotonin syndrome and hyperthermia. Tapentadol is contraindicated during and for 14 days after MAOI therapy.

What this interaction means in plain words

Tapentadol and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Tapentadol belongs to Musculoskeletal; Tranylcypromine belongs to Central Nervous System.

Evidence level: High

Why it happens

Irreversible MAO inhibition plus tapentadol's norepinephrine reuptake inhibition causes excess monoamine activity.

What to do

Do not coadminister, and allow at least 14 days between stopping tranylcypromine and starting tapentadol. Use a non-interacting analgesic.
