Summary

Ritonavir-boosted tipranavir has mixed effects on CYP3A4 and P-glycoprotein, so verapamil concentrations may rise or fall unpredictably. Verapamil may also modestly raise tipranavir levels. Excess verapamil effect can cause bradycardia, AV block and hypotension.

What this interaction means in plain words

Tipranavir and Verapamil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Tipranavir belongs to Anti Virals; Verapamil belongs to Cardiovascular.

Evidence level: Low

Why it happens

Opposing effects of tipranavir on CYP3A4 (inhibition) and P-glycoprotein (induction) alter verapamil exposure.

What to do

Monitor ECG, heart rate and blood pressure when starting or stopping tipranavir, and titrate verapamil to response.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
