Summary
Vilanterol, a long-acting beta-agonist, can lower serum potassium, and torsemide is a loop diuretic that causes potassium loss. Used together they may produce additive hypokalemia and ECG changes, especially if the beta-agonist dose is exceeded or in patients with other risk factors for arrhythmia.
What this interaction means in plain words
Torsemide and Umeclidinium-vilanterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Torsemide belongs to Cardiovascular; Umeclidinium-vilanterol belongs to Respiratory.
Why it happens
Additive hypokalemia from beta-2 stimulated intracellular potassium shift combined with renal potassium wasting caused by loop diuresis.
What to do
Monitor serum potassium and magnesium periodically, particularly in patients with cardiac disease or on higher diuretic doses, and replace deficits as needed. Keep inhaled doses within the labeled maximum.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
