Summary

Tranylcypromine inhibits MAO-A, which contributes to zolmitriptan metabolism, so zolmitriptan and its active metabolite accumulate. Together with additive serotonergic effects this raises the risk of serotonin syndrome and vasoconstriction. Concurrent use is contraindicated.

What this interaction means in plain words

Tranylcypromine and Zolmitriptan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Tranylcypromine belongs to Central Nervous System; Zolmitriptan belongs to Central Nervous System.

Evidence level: High

Why it happens

MAO inhibition reduces zolmitriptan clearance, and both drugs enhance serotonergic neurotransmission.

What to do

Do not combine; allow 2 weeks after stopping tranylcypromine before starting zolmitriptan. Use a non-triptan option for acute migraine.
