Mécanisme d'action
The mechanism in multiple sclerosis is unknown. The active metabolite MMF activates the Nrf2 pathway (cellular response to oxidative stress) and is a nicotinic acid receptor agonist in vitro.
Posologie adulte habituelle (résumé de la notice)
Relapsing forms of multiple sclerosis: starting dose 231 mg orally twice a day for 7 days, then maintenance dose 462 mg (two 231 mg capsules) orally twice a day.

- Unknown Human data come mainly from dimethyl fumarate (same active metabolite): registry data show no rise in birth defects. In rabbits it caused skeletal malformations at higher doses. Use only if clearly needed; a pregnancy registry exists.
- Allaitement
- Safe No direct data, but with dimethyl fumarate the active metabolite reaches milk in low amounts, with no harm in over 20 infants. LactMed: acceptable, at least after 1 month of age. Monitor infant weight gain.

Contre-indications
- Known hypersensitivity to diroximel fumarate, dimethyl fumarate, or any excipient (reactions may include anaphylaxis and angioedema).
- Co-administration with dimethyl fumarate.
Précautions particulières
- Anaphylaxis and angioedema can occur after the first dose or at any time; discontinue and do not restart.
- PML has occurred with dimethyl fumarate (same active metabolite); withhold at the first sign or symptom suggestive of PML.
- Herpes zoster and other serious opportunistic infections can occur; consider withholding treatment until the infection resolves.
