Indications
Mécanisme d'action
Long-acting muscarinic antagonist (anticholinergic) with similar affinity for muscarinic receptor subtypes M1 to M5. In the airways it inhibits M3 receptors at the smooth muscle, leading to bronchodilation.
Posologie adulte habituelle (résumé de la notice)
COPD maintenance: 175 mcg (one 175 mcg unit-dose vial, 3 mL), oral inhalation by jet nebulizer with mouthpiece, once daily
Grossesse et allaitement
- Grossesse
- Unknown Inhaled; no adequate human data. No birth defects or fetal deaths in rats or rabbits at over 200 times human exposure. Women should tell their doctor if pregnant; continue only if the COPD benefit outweighs the risk.
- Allaitement
- Safe Not known if it passes into human milk; rat milk levels were up to 10 times plasma. Only about 3% is absorbed orally, so infant effects are unlikely (LactMed). Long-term use might reduce milk supply; watch the infant's feeding and weight.
Effets indésirables signalés
- Cough
- Nasopharyngitis
- Upper respiratory tract infection
- Headache
- Back pain
- Dry mouth (postmarketing)

Précautions particulières
- Do not initiate in acutely deteriorating COPD or use for acute symptoms or as rescue therapy; extra doses should not be used.
- Paradoxical bronchospasm may be life-threatening; treat with a short-acting bronchodilator, discontinue YUPELRI and institute alternative therapy.
- Use with caution in narrow-angle glaucoma; patients should report symptoms immediately.
