Résumé
Sildenafil greatly potentiates the vasodilator effect of nitrates, which can produce profound, prolonged hypotension, syncope, myocardial ischemia or infarction. Patients with coronary disease who use nitrates are the typical population at risk. Labeling of both drugs contraindicates the combination.

Nitroglycerin and Sildenafil are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Nitroglycerin releases nitric oxide to increase cGMP, while sildenafil inhibits PDE5-mediated cGMP breakdown, causing exaggerated vasodilation.

Do not give nitrates to patients who have taken sildenafil within 24 hours. Patients with chest pain who recently took sildenafil need urgent medical assessment and non-nitrate approaches, and nitrates should be avoided for at least 24 hours after the dose.
