Sintesi

Clarithromycin increases ergotamine concentrations and has been associated with acute ergot toxicity, including severe vasospasm and ischemia of the extremities, central nervous system and heart. Serious ischemic complications have been reported, sometimes after only a few doses. Concomitant use is contraindicated in product labeling.

Cosa significa questa interazione in parole semplici

Clarithromycin and Ergotamine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Clarithromycin belongs to Anti Bacterial; Ergotamine belongs to Central Nervous System.

Livello di evidenza: High

Perché avviene

Clarithromycin strongly inhibits CYP3A4, reducing the metabolism of ergotamine and causing excessive exposure and vasoconstriction.

Cosa fare

Do not combine; choose a non-interacting antibiotic such as azithromycin (weaker effect) or another class suited to the infection if an ergot is needed. Alternatively withhold ergotamine and treat migraine with a non-ergot agent while clarithromycin is given.
