Sintesi
Inhibition of MAO slows catecholamine metabolism and enlarges neuronal stores, and dobutamine is a catecholamine inotrope with beta-1 agonist and some alpha activity. The pressor and chronotropic response to dobutamine can therefore be exaggerated, with severe hypertension, headache and arrhythmia.
Cosa significa questa interazione in parole semplici
Dobutamine and Isocarboxazid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dobutamine belongs to Cardiovascular; Isocarboxazid belongs to Central Nervous System.
Perché avviene
Reduced catecholamine degradation (MAO inhibition) with sympathomimetic stimulation
Cosa fare
Avoid where possible; if dobutamine is required in a patient taking isocarboxazid, start at a markedly reduced dose and titrate against continuous blood-pressure and ECG monitoring.
Cosa chiedere al farmacista
