作用のしくみ
Calcifediol is a prohormone converted mainly in the kidney by CYP27B1 to calcitriol, which binds the vitamin D receptor, increasing intestinal calcium and phosphorus absorption and reducing parathyroid hormone synthesis.
成人の通常用量(添付文書の要約)
Secondary hyperparathyroidism in CKD stage 3 or 4 with 25-hydroxyvitamin D below 30 ng/mL: initial 30 mcg orally once daily at bedtime; serum calcium must be below 9.8 mg/dL before starting
妊娠と授乳
- 妊娠
- Unknown No human data. Rabbits given doses at least 8 times the human 60 mcg/day dose had increased skeletal and soft tissue malformations; none in rats.
- 授乳
- Unknown No information on presence in human milk. Monitor exposed infants for hypercalcemia signs (seizures, vomiting, constipation, weight loss); weigh breastfeeding benefits against maternal need.
報告されている副作用
- Anemia
- Nasopharyngitis
- Increased blood creatinine
- Dyspnea
- Cough
- Congestive heart failure
禁忌
- None
注意が必要な方
- Hypercalcemia and hypercalciuria can occur; acute hypercalcemia may increase risk of cardiac arrhythmias and seizures; chronic hypercalcemia can cause vascular and soft-tissue calcification.
- Hypercalcemia increases the risk of digitalis toxicity; monitor calcium and signs of toxicity more often when starting or adjusting the dose.
- Adynamic bone disease with increased fracture risk may develop if PTH is oversuppressed; monitor intact PTH and adjust dose.
