Unsafe EU/UK labels and the old-format US capsule label contraindicate it in pregnancy (reduced fetal cholesterol synthesis); the current US ER label (2023) says stop when pregnancy is recognised unless the need is compelling. Statin data show no rise in birth defects. Stop before or when pregnant.

Unsafe No human milk data; it passes into rat milk (milk-to-plasma ratio 2:1). US label: contraindicated while breastfeeding because of possible effects on infant lipid metabolism. LactMed: avoid; an alternative is preferred.

  • Lipid-lowering effects additive with bile-acid binding resin or niacin
  • Rare reports of immune-mediated necrotizing myopathy (IMNM), characterized by increased serum creatine kinase that persist despite discontinuing statin
  • Heavy alcohol use, history of liver disease, renal failure

Unexplained muscle pain, tenderness or weakness while taking a statin should be reported promptly. Statins are not used in pregnancy, and some other medicines raise their levels and risk.
