Como funciona
HER2-directed antibody-drug conjugate: a humanized anti-HER2 IgG1 linked by a cleavable linker to DXd, a topoisomerase I inhibitor. After binding HER2, it is internalized and the linker is cleaved by lysosomal enzymes; released DXd causes DNA damage and apoptotic cell death.

HER2-positive, HER2-low or HER2-ultralow metastatic breast cancer, HER2-mutant NSCLC, HER2-positive (IHC 3+) solid tumors: 5.4 mg/kg IV infusion every 3 weeks as monotherapy until disease progression or unacceptable toxicity

- Unsafe Can cause fetal harm; HER2-directed antibodies linked to oligohydramnios, pulmonary hypoplasia, skeletal abnormalities and neonatal death. No human data.

- Unsafe No human milk data; advise not to breastfeed during treatment and for 7 months after last dose.

Advertências de caixa preta
- Interstitial lung disease and pneumonitis, including severe, life-threatening and fatal cases, occur. Monitor for and investigate cough, dyspnea, fever and new respiratory symptoms. Permanently discontinue in all patients with Grade 2 or higher ILD/pneumonitis.
Quem deve ter cuidado
- ILD/pneumonitis: monitor, evaluate with imaging, give corticosteroids; withhold or permanently discontinue per grade.
- Neutropenia including febrile neutropenia: monitor CBC before initiation and before each dose.
- Left ventricular dysfunction: assess LVEF before initiation and at regular intervals; not studied with LVEF less than 50%.
