Resumo
Combining a direct thrombin inhibitor with a potent P2Y12 antiplatelet markedly raises the risk of major bleeding, including gastrointestinal and intracranial hemorrhage. Prasugrel carries its own high bleeding risk, particularly in older or low-weight patients.
O que esta interação significa em palavras simples
Dabigatran etexilate and Prasugrel can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dabigatran etexilate belongs to Cardiovascular; Prasugrel belongs to Cardiovascular.
Por que acontece
Additive effects on hemostasis: dabigatran inhibits thrombin while prasugrel irreversibly inhibits platelet P2Y12 receptors.
O que fazer
Use only when clearly indicated and for the shortest duration; consider a less potent P2Y12 inhibitor such as clopidogrel and add gastroprotection. Monitor hemoglobin and for bleeding signs.
